COVID-19 Case Management
Thank you to those practices who have started providing COVID-19 care for their patients. Please assess and then focus care on those who are more unwell or at higher risk of more severe COVID-19 to identify clinical deterioration early – see HealthPathways COVID-19 Case Management in Adults.
If you are having difficulty accessing CCCM records it may be because the person is still in the process of contact tracing. The Whānau HQ team can assist. Please text or phone Whānau HQ Ops: 021 1907 697 or email whānauhq@adhb.govt.nz.
Higher acuity referral to Whānau HQ hub for weekend calling
The Whānau HQ hub can support clinical review outbound calls on weekends and public holidays, on behalf of general practice, to more moderately unwell patients or people at higher risk – usually higher acuity cases (5-6). Please raise a yellow flag in CCCM if your patient needs this – see HealthPathways COVID-19 Case Management in Adults.
Most cases can isolate safely at home
A very small number of cases may need to go into MIQ if they have no place to isolate safely. Current criteria for MIQ is:
· If the case is experiencing homelessness or living in residential housing setting with shared facilities, MIQ may be appropriate. These cases can be escalated to ARPHS.
· Everyone else – even those pregnant, with underlying medical conditions and who are unvaccinated – should be able to isolate at home with support from Whānau HQ.
However, Whānau HQ is actively working on sourcing alternative accommodation options to support people who are not able to isolate at home for safety or other critical reasons.
